Thera Vitality Improvement Program
Terms and Conditions
Thera-VIP, which stands for Vitality Improvement Program, is intended to encourage people who have experienced the beneficial effects of progressive massage therapy, to make it a continuing and regular part of their wellness program, by making it affordable as well as customized to the particular and changing needs of each member.
A. Membership
1. Applicant must be at least 18 years old at the date of application.
2. TheraV Family Massage, hereinafter referred to as TheraV, reserves the right to accept or reject any application to this Program.
3. Membership to this Program shall be for a minimum of 12 member-months, and shall take effect upon approval of this application by TheraV Family Massage, hereinafter referred to as TheraV, and receipt of the initial monthly membership fee. The “member-month” in this context is reckoned from any chosen day of the current month until the same day of the following calendar month, e.g. Feb 12-March 12, or March 28-April 28. For this application, the monthly membership fee for the first 12 months of membership shall be in the amount of $79.00/month.
4. After the first 12 member-months mentioned in Par 3 above, such membership shall automatically continue indefinitely on a month-to-month basis until terminated in writing or by email by either the Member or by TheraV Family Massage.
5. Should the Member want to cancel this Membership within the above-mentioned first 12 member-month period, Member shall have the obligation to pay a Cancellation Fee of $100. Any unused loyalty massage credits are subject to forfeiture upon cancellation of membership.
6. This membership is personal to the named Member, and is non- transferrable.
7. For purposes of identification and billing, the Applicant/Member shall provide TheraV with current, accurate, complete and updated personal information including name, residence address, email address, telephone number, and applicable payment data, as well as any changes that may occur thereafter.
8. TheraV reserves the right to review this undertaking periodically. The Member shall be notified by email regarding any changes which may include, but not be limited to, changes in the amount of the membership fee or date of automatic payment of the same.
B. Member Benefits
1. Corresponding to the automatic monthly membership fee, the Member shall be entitled once a month to a 60-minute Loyalty Massage. This may be rolled over to the next month. Alternatively, the Member may assign this Loyalty Massage benefit to another person upon approval by TheraV. If such Loyalty Massage credits are rolled over and allow to accumulate, the Member may use them or assign them to be used by another person or persons provided that any one person may use only one Loyalty Massage credit in any given day.
2. All other massage services during the month in addition to the 60-minute Loyalty Massage mentioned in Par B-1 above, shall be at a specially discounted Member Rate shown below. Such rates maybe adjusted from time to time at the sole discretion of TheraV. For the guidance of all concerned, please refer to the rates below. These rates will always be current and effective at any point in time.
C. Automatic Payment
1. The amount of the monthly Membership Fee shall be determined in consultation with TheraV at the time of application. Such amount shall be level for the first 12 months of membership. Thereafter, any change in the amount shall be communicated by email to the Member 30 days before such change takes effect.
2. The Applicant authorizes TheraV to effect automatic monthly deduction of the membership fee from his/her credit/debit card on the first day of each member -month.
D. Rates
